The article does not recognize another fundamental distinction -- voluntary or coerced. Also, “actuarial” insurance is "social", as he uses those terms, to some extent. Within any given rate class in “actuarial” insurance, there will be some insureds who have claims and others who don't. So there is redistribution from the latter to the former.

Later he addresses Medicare claim payments. Does he think the amounts Medicare pays are voluntarily agreed to by the health care providers? The health care providers can complain and lobby, but Medicare decides by itself how much it pays for the most part.

I think the first thing to notice here is that the US health care system is far from capitalistic, as it is rife with coercive government intervention. It is immoral in forcing one person to provide, or pay to provide, another's medical care (as in government health insurance or hospitals); it is immoral in forcing patients to abstain from experimental procedures or drugs they wish to try (as in FDA regulations); it is immoral in forcing doctors to not prescribe drugs they and their patients deem useful (as with narcotics and their levels that government officials don't approve of), and on and on. So much coercive, non-capitalistic, rights-violating intervention in what could otherwise be a peaceful, productive, rights-respecting arena.

It is unfortunate that most voters are so concrete-bound that they gladly point to concrete precedents of government programs to justify more government programs. This mentality, combined with the so-called "universal suffrage" franchise, practically guarantees growth in government. I see no way out of this mess.
