In fag parlance, to "hook-up" originally meant to sleep with someone you've just met, usually from a bar or online. It can now mean sleeping with someone at the end of a date. (e.g., "How was your date last night? Did you hook up?")
Usually if you hook up with somebody it means it was a one-time thing. ("To trick" is used in this same way; hence seeing someone out at the bar or club that your friends haven't met before, and explaining when the guy walks away "Oh, he was just a trick." And no, it has none of the prostitution connotations that it has for the straight world.)
You know, I feel a little funny relating this. Like I'm giving away secrets from the clubhouse. Oh, well. If anyone wants to join, the initiation is a scream. So to speak.
